Pre-Register for an MLD Class
Use the form below to pre-register for an upcoming class. Please fill out all fields. Nicola McGill or Geri Ruane will contact you with payment options and more information upon receipt of this form.
PLEASE NOTE : Only healthcare professionals with a current active license are eligible to take this MLD Certification class. This pre-registration form does NOT secure your place in the class of your choice. Once you have completed this pre-registration form, you will be contacted by email with registration details requesting that you provide the required paperwork and payment of tuition. ($1300) Once you make your tuition payment, your place in the class is secure.
If a registration email is sent to you and you don’t respond within 5 days, you may lose your spot in the class. Your prompt attention to completing these steps is very much appreciated.
